Item High tibial osteotomy for medial compartment osteoarthritis: A comparison of clinical and radiological results from closed wedge and focal dome osteotomies(Sage Publications, 2007-09-11) Bilgen, Muhammet S.; Atıcı, Teoman; Bilgen, Ömer Faruk; Uludağ Üniversitesi/Tıp Fakültesi/Ortopedi ve Travmatoloji Anabilim Dalı.; A-5095-2018; 35723877700; 7801647288; 16933501900This study aimed to evaluate the clinical and radiological results of closed wedge osteotomy (11 knees) and focal dome osteotomy (14 knees) in cases of high tibial osteotomy undertaken for varus knee with medial compartment osteoarthritis. Clinical evaluation was performed using the Knee Society Score and no significant difference was seen between the two groups at final follow-up. Radiological evaluation was made on the basis of the pre- and postoperative mechanical axis, postoperative movement of the tibial axis, loss of correction at final follow-up and patellar height measured using the Insall-Salvati index. Statistically significant differences were seen with focal dome osteotomy compared with closed wedge osteotomy in the Insall-Salvati index at final follow-up, the amount of correction loss and the change in tibial axis location. This article presents a case of malignant transformation of monostotic fibrous dysplasia into angiosarcoma. An 80-year-old female presented with pain on right cruris. Radiological examination revealed a lesion with lytic areas and destruction of cortical bone on right tibia. Gross and histopathological examination showed two areas with an abrupt transition. The solid component was composed of curved, immature bony trabeculae in a fibroblastic stroma. The other component involved epitheloid cells forming slit-like vascular spaces. The diagnosis of angiosarcoma and fibrous dysplasia was given. Malignant transformation of fibrous dysplasia into angiosarcoma is extremely rare; as this is the sixth case in the existing literature. Prognosis of fibrous dysplasia is generally good and less than 1% of the patients develop a malignant tumor. Therefore, patients with fibrous dysplasia should be offered a life-long follow-up.
