Although he is not well known at his living era, he succeeded to publish the first Neurosurgery Journal in Turkish, leading neurosurgery to progress in Turkey. After his surgery education in Germany and France, he practiced in Istanbul and Adana. In majorty of his practice he was dealed with neurosurgery but later he was interested into the Plastic Recontructive and Aesthetic Surgery and performed many operations on this branch. At 1946 he published a book "Plastic Reparative and Aesthetic Surgery" about his workings and cases. The majority were flame burns, followed by scalds. The male to female ratio was 3: 1. More than half of the patients were adults (more than the age of 16 years) and most of the injuries occurred at home. In the group of patients under 2 years of age, scalds were the leading cause of burns and among them hot tea spill was the most common reason. Many of the electrical burns occurred at the workplace and at home were resulted from inappropriate close location of high voltage lines to constructions and houses, and inappropriate use of paint thinner was found to be an important cause for flame burns occurred at the work place. The mean hospitalization time was 27.1 days and the overall mortality rate was 14% (n=54). Physical examination showed pulsatile mass at hypothenar region. After the description of the gluteal perforator-based flap, the perforator-based fasciocutaneous flap became popular in lumbosacral tissue defect reconstruction.Methods Multiple perforatory preserving superior gluteal artery perforator (SGAP) flap was performed in eight patients with lumbosacral soft tissue defects from May 2014 to April 2015. Five men and three women received the operation. The causes of defects were pilonidal sinus defect in five patients and pressure ulcer in three patients. All patients were mobile and did not have any systematic diseases.Results The average age of the patients was 45 years. The follow-up period was 2 to 12 months (mean, 6 months). The size of the flap ranged from 8x10 to 10x16 cm. All flaps healed completely without any vascular problem. Only one patient suffered from dehiscence. The average operative time was 160 min (range 120-200 min). Patients were immediately allowed to lie on the operated area (but no longer than 1 h at a time for the first 48 h). They were allowed to gradually sit after 72 h. The mean time for removal of suction drains was 8 days (5-14) while mean duration of hospital stay was 7.5 days (3-14 days).Conclusions It was seen that multiple perforatory preserving SGAP flap is a safe and reliable flap. No vascular complications was seen in postoperative period. Preserving multiple perforators in SGAP flap is time-consuming and increases the necessary time for flap harvest which is the main drawback of our technique.Publication Outcomes of elderly burn patients requiring hospitalization(Taylor ve Francis, 2015-06-01) Şimşek, Muhammed Eren; Özgenel, G. There is limited data about the outcomes of elderly burn patients requiring hospitalization.Methods: In this retrospective study, we assessed the epidemiology and outcomes of burn injuries in elderly patients (>60 years old) admitted to a burn unit of a tertiary medical center based on patient characteristics, type and extent of burns, treatment, hospital stay and mortality rates.Results: Forty-eight elderly burn patients among 870 burn patients during the study period were evaluated. Fire was the most common cause of burns (77.1%). Most of the burns involved more than 20% of total body surface area. Twenty-six (54.2%) patients died during hospitalization. Although burn surface area slightly and non-significantly increased in patients over 75 years, there was a significantly increased mortality rate in these patients. Multivariate linear regression analysis revealed burn area and age as independent associates of mortality.Conclusion: Our data show a high mortality rate in elderly burn patients. While its prognosis is generally favorable, disease recurrence is relatively frequent. Since morbidity after repeated surgery may be significant, an optimized prediction of recurrence-free survival (RFS) has the potential to improve current management strategies. The purpose of this study was to investigate the prognostic value of the Ki-67 proliferation index with respect to RFS in patients with DFSP We retrospectively analyzed data from 45 patients with DFSP. We calculated the Ki-67 proliferation index as the percentage of immunostained nuclei among the total number of tumor cell nuclei regardless of the intensity of immunostaining. We constructed univariate and multivariate Cox proportional hazards regression models to identify predictors of RFS. Among the 45 patients included in the study, 8 developed local recurrences and 2 had lung metastases (median follow-up: 95.o months; range: 5.2-412.4 months). The RFS rates at 60, 120, and 240 months of follow-up were 83.8%, 76.2%, and 65.3%, respectively. The median Ki-67 proliferation index was 14%. Notably, we identified the Ki-67 proliferation index as the only independent predictor for RFS in multivariate Cox proportional hazards regression analysis (hazard ratio = 1.106, 95% confidence interval = 1.019-1.200, p = 0.016). If this condition becomes chronic, surgical treatment is the only option. The goal of surgical treatment is to reposition the condyle and prevent further recurrences. In this study, a retrospective analysis is presented examining 73 patients and 83 joints that were treated with hook-shaped miniplates and miniscrews fixed to otolog bone grafts. Records were obtained from archives of the Uludag University Medical Faculty, Plastic, Reconstructive and Aesthetic Surgery Department. The duration of postoperative follow-up was 3-30 months. In one case, the miniplate was fractured, and in two cases, abscesses existed. This study aims to emphasize that using miniplates and otolog bone grafts is more cost effective.
