Abstract
Sağlık hizmetinin kaliteli olmasında modern tıbbi bakımın gerektirdiği en iyi hizmetin verilmesi kadar, hastaların hak, gereksinim ve beklentilerinin de karşılanması gereklidir. Çalışmamızda; Bursa'da bulunan hastanelerde çalışan uzman hekim ve tıpta uzmanlık öğrencilerinin hasta hakları konusundaki bilgi düzeylerinin/görüşlerinin değerlendirilmesi amaçlanmıştır.Ankette bulunan hasta hakları ile ilgili maddelere uygulanan faktör analizi sonucu 3 faktör bulundu (''bilgilendirme hakkı'', ''izin ve saygı hakkı'' ve ''seçme hakkı'' faktörü). Günlük hasta sayısı azalması ve bir hasta için ayrılan sürenin artması ile hekimlerin tüm faktörlere daha fazla katıldıkları tespit edildi. Ayrıca hekimler, hastaların fazlalığı nedeniyle hastalara yeterli süre ayrılmadığı ortak düşüncesini belirtmişlerdir. Hekimler için en önemli 3 hasta hakkı, hastanın tanısı, tedavisi ve hastalığının prognozu konusunda yeterince ve anlayacağı dilde bilgilendirilmesi olarak sıralanmıştır. Hekimlerin %28.1'i hasta hakları yönünden öncelik taşıyan hasta grupları olmadığını bildirirken,olduğunu bildirenlerin %34.2'si sadece acil hastaların öncelik taşıdığını bildirmiştir. Hekimlerin hasta hakları ile ilgili bilgilerini çoğunlukla mezuniyet sonrası eğitimlerden aldıkları saptanmıştır. Hekimlerin %23.8'inin meslek hayatları boyunca en az 1 kez hasta hakları konusunda şikayet edildiklerini bildirdikleri tespit edilmiştir. Meslek sürelerine ve unvanlarına göre farklılıklar olsa da hekimler, Hasta Hakları Yönetmeliği, Hekim Etik Kuralları Yönetmeliği ve Yeni Türk Ceza Kanunu'nun hekimlik pratiği ile ilgili maddeleri hakkında yeterince bilgili olmadıklarını düşündüklerini bildirmişledir. Sonuç olarak, hekimlerimizin çalışma iyileştirilmesi, şartlarının hastalara daha fazla zaman ayırması için gerekli önlemlerin alınması, hasta hakları ve bu konuda meydana gelecek olumsuzluklar sonucu ortaya çıkacak yaptırımlar konusunda hekimlerin bilgilendirilmesi ile hasta ve hekim memnuniyetinin daha üst seviyede olacağı bir sağlık sisteminin oluşacağına inanıyoruz.
The quality of health care facilities depends on providing standardmodern medical care as well as satisfaction of patients' rights, requirementsand expectations. In our study, we aimed to evaluate the current status ofmedical knowledge/opinions of the medical specialists and residents workingin Bursa hospitals. Out of many questionnaire items concerning patients' rights, threefactors were identified in factor analysis (''information right'', ''permission andrespect right'' and ''selection right'' factors). It was found that the decrease inthe daily amount of patients and the increase in time needed to be spend foreach patient resulted in physicians' more agreement for all factors. Inaddition, physicians have reported a consensus of opinion about spendinginsufficient time because of the excess number of patients. The three mostimportant patient's rights according to the physicians were listed asacknowledgement of the patient about the diagnosis, treatment, andprognosis of the disease in a sufficient and easily understandable manner.While 28.1% of the physicians reported that there was no specific group ofpatients that had priority about patient's rights, of the remaining physicianswho made positive declaration 34.2% reported that only the emergencypatients had priority. It was found that most of the physicians have gainedinformation about the patient's rights through postgraduate education. It wasfound that 23.8% of the physicians have reported being complained aboutpatient's rights, at least once throughout their professional career. Althoughthere were differences between practice duration and title of the physicians,they have stated that they did not think they had adequate information about Patient Rights Directives, Physician Ethical Rules Regulation and New Turkish Penal Code articles concerning physicians' practices.In conclusion, we believe that improvement of the physician's workingconditions, implementation of necessary measures so that the physicians canspend more time with their patients, and informing of the physicians bothabout patient's rights and the consequences of violating them, may contributein forming a superior health system where both physician and patientsatisfaction are optimized.
