Abstract
Köpeklerde farklı yöntemlerle yapılan Juvenile Pubik Simfizyodezis’in (JPS) karşılaştırmalı araştırılması amaçlanmıştır. Belçika Malinois ırkı, 16-20 haftalık, 18 adet sağlıklı dişi köpek yavrusu kullanıldı. Klinik ve ortopedik muayenede topallıktan şüphelenilen ve radyolojik muayenede distraksiyon indeksi (DI) ≥ 0,4 olan köpekler çalışmada kullanıldı. Üç grupta planlanan çalışmada: grup I’de (GRI, n=6) diyot lazer, Grup II’de (GRII, n=6) kriyocerrahi ve Grup III’te (GRIII, n=6) elektrokoter ile JPS gerçekleştirildi. Pre- ve postoperatif klinik, ortopedik ve radyolojik muayeneler yapıldı. IL-1β ve MMP-3 analizleri gerçekleştirildi. Verilere istatistiksel analiz yapıldı. Postoperatif Ortolani testinde; sadece GRIII’te 1 köpekte bilateral eklem laksitesi vardı. Barlow testinde; GRII ve GRIII’te 1’er köpekte bilateral eklem laksitesi görüldü. Postoperatif GRI’de 1 ve GRII’de 2 köpekte DI değeri 0,4 ve diğerlerinde 0,4’ün altında, GRIII’de 4 köpekte 0,4’ün altındaydı. Postoperatif Norberg Açısı (NA) değeri GRI’de tüm, GRII’de sağ 5 ve sol 4, GRIII’te sağ 4 ve sol 5 köpekte 105°’nin üzerindeydi. Federation Cynologique Internationale (FCI) skorlandırmasında preoperatif 14 köpekte “C” 4 köpekte “B” iken, postoperatif 12 köpekte “A” 6 köpekte “B” sınıfı olarak saptandı. Preoperatif sağ kalça eklemi Barlow testi bulguları için gruplar arasında farklılık vardı (p=0,025). DI ve sağ ile sol kalça eklemlerinin NA değerleri için tüm gruplarda ölçüm zamanları yönünden fark anlamlıydı (p<0,05). IL-1β için grup içi karşılaştırılmada sadece GRI’de (p=0,001); MMP-3 için GRI ve GRIII’te anlamlı fark görüldü (p=0,000). Histolojik olarak operasyon bölgesinde tüm gruplarda kıkırdak ve bağ dokusu oluşumu saptandı. GRII’de bağ doku ve kollajen iplikler yoğundu. Sonuç olarak, yavru köpeklerde yapılan JPS’nin kalça displazisinin ilerleyici klinik, ortopedik ve radyolojik bulgularını önlediği; IL-1β, MMP-3 düzeyleri ile histolojik bulgular temelinde JPS için yöntem olarak kriyocerrahinin daha olumlu bulgular sunduğu görülmektedir.
A comparative study of Juvenile Pubic Symphysiodesis (JPS) performed with different methods in dogs was aimed. 18 healthy female puppies of Belgian Malinois breed, 16-20 weeks old, were used. Dogs with suspected lameness in clinical and orthopedic examination and with a distraction index (DI) ≥ 0.4 in radiological examination were used in the study. In the study planned in three groups: JPS was performed with diode laser in group I (GRI, n=6), cryosurgery in Group II (GRII, n=6) and electrocautery in Group III (GRIII, n=6). Pre- and postoperative clinical, orthopedic and radiological examinations were performed. IL-1β ve MMP-3 analyzes were performed. Statistical analysis was performed on the data. In the postoperative Ortolani test; only 1 dog in GRIII had bilateral joint laxity. In the Barlow test; bilateral joint laxity was observed in 1 dog each in GRII and GRIII. The DI value was 0.4 in 1 and 2 dogs in GRII and below 0.4 in the others in postoperative GRI and below 0.4 in 4 dogs in GRIII. Postoperative Norberg Angle (NA) value was above 105° in GRI, right 5 and left 4 dogs in GRII, right 4 and left 5 dogs in GRIII. In the Federation Cynologique Internationale (FCI) scoring, it was determined as “C” in 14 dogs preoperatively and “B” in 4 dogs, while “A” in 12 dogs and “B” in 6 dogs postoperatively. There was a difference between groups for preoperative right hip Barlow test findings (p=0.025). In all groups, the difference between the measurement times before and after JPS was significant for DI and NA values of the right and left hind extremities (p<0.05). For IL-1β, in-group comparison only in GRI (p=0.001); There was a significant difference in GRI and GRIII for MMP-3 (p=0.000). Histologically, cartilage and connective tissue formation was detected in all groups in the operation area. Connective tissue and collagen fibers were dense in GRII. In conclusion, JPS in puppies prevented progressive clinical, orthopedic and radiological findings of hip dysplasia; on the basis of IL-1β, MMP-3 levels and histological findings, it seems that cryosurgery as a method for JPS offers more positive findings.
