Abstract
Türk halkı arasında ‘delibal’ olarak bilinen ve Nepal, Brezilya, Japonya ve Türkiye’de de Karadeniz bölgesinde üretilen balda bulunan, Rhododendron ponticum isimli nektarın, şiddetli bradikardi ve hipotansiyon yaptığı bilinmektedir. Bu bal Rhododendron cinsi bitkilerin iki üyesi olan R. Luteum ve R. Ponticum’den Türkiye’nin kuzey bölgelerinde üretilmektedir. Grayanotoxin I, diğer adıyla andromedotoxin yalnızca Ericaceae bitkileri tarafından oluşturulmaktadır ve toksikasyondan sorumlu içerik olarak kabul edilmektedir. Tipik zehirlenme semptomları genellikle gastointestinal olup bazen yaşamı tehdit eden şiddetli bradikardi ve hipotansiyon gelişebilmektedir. Bizde son beş yıl içinde kliniğimize, Karadeniz Bal’ı yedikten sonra tipik zehirlenme semptomları göstererek başvuran tek hastayı sunduk. Karadeniz Bal’ı (delibal) zehirlenmesi oldukça nadir olmakla beraber erken farkedilip müdahale edilmezse ölümcül bir tabloya dönüşebileceği akılda tutulması gereken bir durumdur.
The honey which is produced in Japan, Nepal, Brazil and the Karadeniz Region of Turkey is also known as the ‘mad honey’ by Turkish people and contains Rhododendron ponticum nectar, which is known to cause severe bradycardia and hypotension. This honey is produced by Rhododendron species R. luteum and R. ponticum in the northern regions of Turkey. Grayanotoxin I also known as andromedotoxin, is only produced by Ericaceae plants and thought to be the compound which is responsible for the poisoning. Typical toxication symptoms are gastrointestinal and sometimes may cause life-threatening severe bradycardia and hypotension. Here we present the only patient in five years who attended to our clinic with the typical symptoms of toxication after the ingestion of Black Sea honey. Although the Black Sea Honey (Mad Honey) toxication is very rare, it is a situation to keep in mind that if it is not treated by early diagnosis it can turn into a mortal state.
